| Nobody's record is pristine. Knowing how insurance companies work, I expect any coverage would be pretty damn expensive. Maybe the payload would be worth covering, but I doubt the launch vehicle is. (The rocket costs $62m, the payload this time cost around $300m - quite a difference.) |

| Most of these launches do have insurance, but it is very expensive. Duh!

| Anomaly is a term of art for us quality control nerds. A test has expected results. If you get something unexpected, that's an anomaly, which may or may not be caused by a bug (maybe the test case was defined incorrectly). |

| I'm familiar with the word anomaly - it is just such a pussy way to describe a giant explosion. "Major Fuckup in Launch" would be better. |
